Biometric Screening

The Clinic Connection™ utilizes biometric screening to meet multiple goals, including both tracking and diagnosing chronic disease in members, to assist with both lifestyle improvements or adjustments, and cost containment for the health plan. On a yearly basis, biometric screenings are conducted at an employer’s selected location, at which a venipuncture blood draw is taken. This allows us to create a roadmap to wellness for a patient’s health. Based upon the results of this screening, The Clinic Connection™ will conduct robust outreach efforts to actively engage patients in using the clinic to both monitor and improve their results, and to build a relationship based upon their individual results and goals for chronic disease management.
