About Clinic Connection

What we are: The Clinic Connection™ is a partnership of local hospital systems providing Chronic Disease Management (CDM) and lifestyle support clinic models for all sized employers.

 

What we do: We work with employers who want to manage costs while improving long-term health outcomes for their employees. We work with hospitals and providers who want to help their communities with extended access to quality services, close to home. Employee outreach is at the forefront of our clinic model; we actively communicate with employees to engage them in utilizing their clinic. Our focus is on chronic disease management, achieved through working with employees to manage and improve their overall health at home, at work, and at play.

Patient Success Stories

Clinic Patient 1 is a true success story. He came to the Clinic for routine biometric testing to obtain a snapshot of his overall health and a baseline for goal setting. A blood draw was taken, and a full panel was run for this patient. The results of the panel indicated blood levels that were suspicious, and as a result, the patient was contacted by the Clinic doctor, and was told he needed to be seen again for additional testing. Upon receiving the results of this additional testing, it was determined that he had a mass on his liver. This patient was then referred on for further treatment to address this life-threatening condition, and his treatment has been successful. 

Participating in the Clinic’s basic services resulted in catching his very serious health issue in the early stages, thereby greatly increasing the likelihood of success in treatment, as well as reduced costs for all involved.

Patient 1 said “You saved my life! I am so thankful for the clinic staff who followed up on my results to determine what was causing the problems. Without their insistence that I have additional testing, I don’t know what would have happened. Thank you!” Patient 1 is an outstanding example of how using the Clinic for what would be considered routine testing can result in highly successful outcomes.

Clinic Patient 2 was a more classic example of a patient for whom having routine medical exams, or seeking healthcare of any kind, was not a priority. He in fact boasted about “not having been to a doctor in 17 years!” His streak of not having any kind of medical care ended, however, when he participated in biometric screening at the Clinic. As part of the routine screenings given during the biometric exam, it was determined that this patient’s blood pressure was 200/100. As a result of that very negative result, this patient was referred that same day by the attending physician at the Clinic to a doctor who immediately began treatment that same day to lower this patient’s blood pressure

Because high blood pressure is a significant factor in a patient’s overall health, treating this condition was a critical component of his chronic disease management, along with improving his long-term outcomes. This patient continued with follow-up testing at the Clinic over time to monitor his blood pressure and the effectiveness of the medication being used to control it. An additional benefit was an improvement in Patient 2’s overall attitude toward healthcare, increasing the perceived value of participating in routine biometric screening available through the Clinic. He’s had a change of heart and is now healthier and happier as a result.

Clinic Patient 2

Clinic Patient 3  first came to the clinic for health coaching sessions in August 2018. At that time, he had already made big changes to his health on his own. He quit a 1-1½ packs-a-day smoking habit cold turkey, and was 18 months smoke free. He’d already lost 60 lbs. on his own by cutting back on the quantity and frequency of fast food, processed foods and beer, but he was stuck and the scale wasn’t moving anymore. He had difficulty doing simple tasks, and said he was often out of breath. With the assistance of the Health Coach, he learned what a healthy diet should look like and what foods feed his energy and his health. He started incorporating exercise into his routine as well as focusing on healthier food choices. He comes to the Clinic monthly for an InBody scan to check his progress, even though he lives and works over an hour away from the Clinic. 

Patient 3 began his health journey because he had a one-year-old son that he wanted to play with, be a good role model for, and he also wanted to live a long, health life. As of his last appointment, he was 120 lbs. lighter than when he started this journey. He now runs nearly every day, has energy that he didn’t realize was possible, and his everyday life is much easier. He is very close to weighing less and being healthier than he was in high school, and he now has a very positive long-term outlook for maintaining that good health long term.

It allows employees access to free medical personnel, free labs, and free prescription drugs for a pathway to better lifestyle management, under- standing and control of their chronic disease​. Your employees will be seen by appointment only, ensuring that they not only receive treatment in a timely manner, but also that their visit will be efficient and impactful.

A full-time clinic management consultant and clinic operations administrator are available for support, biometric and company wellness events, problem resolution, billing questions, and other administrative duties to support clinic operations. Clinic marketing materials can be prepared and customized to meet your specific needs.
